About DAVE
The Database Application for Vital Events, known as DAVE, consists of (4) four different modules, the Electronic Death Registration System, the Electronic Fetal Death Registration System (EFDRS), the Electronic Birth Registration System (EBRS) and the Electronic Marriage Registration System (EMRS). Records of death, fetal death, birth and marriage may be created, registered, and issued from the system. DAVE contains:
- Deaths from January of 2011 to Present
- Fetal Deaths (coming soon)
- Births from September of 1995 to Present
- Marriages from January of 2017 to Present
What is the URL for the DAVE System?
Database Application for Vital Events (DAVE)
Health care providers and staff at any Maine hospital may login to the DAVE system to create (or find and complete) a birth or death record. Funeral directors and staff at any Maine funeral establishment may login to create (or find and complete) a death or fetal death certificate. Municipal clerks and staff at any municipality in Maine may login to create a marriage license and a marriage record as well as issue any death, fetal death, birth, or marriage certificate if the event took place in their municipality or when a resident was living in that municipality at the time the death, fetal death, marriage or birth occurred.
How do I become a user of the DAVE system?
Maine health providers and staff at the hospitals, funeral directors and staff at the establishments, municipal clerks and staff appointed by the municipal clerk, may register for an account to become a user of the DAVE system by completing the enrollment forms below.

Who do I call or e-mail if I need help with the DAVE system?
The help desk may assist with usernames and passwords, error overrides, registering for a DAVE account, orders for safety paper, checking the status of VitalChek or mail orders, funeral home orders, and case management. Please contact the DAVE Application Support and Help Desk at 1-888-664-9491 (Option 7) or (207) 287-5447. Users may also e-mail the help desk at edrs.dhhs@maine.gov.
