Home Visitor Lead Dust Testing
A Message for Home Visitors
Thank you for participating in the effort to help families with children at risk for lead poisoning take steps to identify lead dust in the home and encourage parents to have their children screened for lead poisoning. Online Training
Before you begin helping families do a lead dust test, you must take this recorded training. The training is 35 minutes long and will provide you with basic information about childhood lead poisoning in Maine and will teach you what you need to know about testing for lead dust so that you can integrate home lead dust testing into your home visits.

Quarterly Narrative Reports
Narrative reports should include a description of progress made toward meeting contract deliverables and must be accompanied by a materials use form to track your distribution of small media materials. Please use the reporting forms below for your reports.

- Workplans are due annually on December 15 utilizing this Tool.
Community Capacity Self-Assessment Tool
All lead poisoning prevention partners are required to complete and submit a community capacity self-assessment annually. Please use one of the tools below to complete your assessments. Choose the One City version of the tool if you only provide services in one city or if you would like to do a combined assessment of the two cities you serve. Choose the Two Cities version of the tool if you would like to assess capacity in two cities separately. Completed self-assessments are due annually on September 30. Send completed self-assessments to Erin Guay and Erin Arneson.
