    • Approved Program Verification  (PDF) This verification form is to be completed by the college where the state approved program was completed. 
    • Waiver Affidavit. (DOC, 37KB) The waiver affidavit form needs to be submitted to the Certification Office by the Superintendent only, if a waiver is required to hire a teacher who is ineligible for a certificate.
    • Ed Tech Upgrade Form (EXCEL, 11KB). Fill out and send in this form with Application ET Upgrade
    • Teacher Rubric (PDF) This form is for those who choose option 3 for meeting the praxis requirement.  It is important to note that this portfolio only meets the praxis requirement, and not the coursework requirement. 
    • 093 School Psychologist Application Packet (PDF)- This packet is for those applying for the 093 endorsement only.
