Email, Paper, PDF/Zip File, or thumb-drive submissions will no longer be accepted.
Entities MUST: Initiate, Renew and Pay Online to apply or continue your license for one year. You will still receive your annual renewal notification, but Invoices will no longer be sent.

The Maine Bureau of Insurance is providing copies of what appears on the online application and corresponding General Instructions, so you can prepare all pertinent information and documentation prior to initiating your application online. Please note these application requirement documents are not to be used as substitutions for the electronic forms online.

Renewal Information
Maine's Utilization Review licenses are continuous, meaning the license renews annually every April 1st. Updated license certificates are not issued again at renewal as there is no expiration date. Please refer to our ALMS Online/Licensee Search to view your entities issue and expiration dates.
Renewal notices go out prior to April 1st asking if the URE wishes to renew its license in Maine for the coming year. The URE must submit their renewal application and $100 filing fee on-line. Any on-line application received after April 1st will be charged a late fee.
All renewal submissions for 2025 require a full and complete submission. In the past, entities were allowed to answer “no changes” but beginning in 2025 submissions must be in full every five years. Please provide a summary of all changes since the effective date of the entity's current license and/or the prior year's filing to continue its license for another year, or a red-lined version of the policies.
If the URE wishes to terminate their Maine license, just return the notice via email, and request termination to Violet Hyatt at violet.m.hyatt@maine.gov.
For annual renewal applications, please provide redlined versions of all document changes submitted for questions 4-9.
Please mark all materials as numbered exhibits.
Note: At any time before or after initial MUR licensing or license renewal, MURs must furnish all medical utilization review criteria to the Bureau of Insurance or the Maine Department of Human Services upon demand.
Material Changes or Additions: It is the responsibility of the external review entity to notify the Bureau of Insurance within 30 days of any changes in the information provided with this application.
Insurance Administrator Registration: If the applicant receives or collects charges, contributions or premiums, or adjusts or settles claims on residents of this state, the applicant must apply for a Third Party Administrator license pursuant to the requirements of Title 24-A M.R.S.A. §1901 et. seq. Update License Information
Effective Immediately:
Utilization Review entities must also update the following information on-line by clicking on the following link: ALMS Online
- Update license Information - Change your license information including your contact information, affiliations, responsible individual, Service of Process contact, and other company contacts as applicable.
- General information Submittal - Select this option to upload information relating to changes in officers/owners and submission of material change reports.
- Request a copy of my license - Individuals and Business Entities Only Select this option to retrieve a copy of your license.
All contact information must be updated by 12/31 each year to receive timely renewal notices via email.

Pursuant to Title 24-A §2771, utilization review entities are required to submit the initial license fee and renewal fees made payable to: Treasurer, State of Maine.
Initial Fee: $400
Renewal Fee: $100
Renewal and Payment Due: 4/1 each year
Late Fee: $25

Statutory Requirements
Title 24-A M.R.S. Chapter 34 requires Utilization Review Entities to be licensed in Maine and must also comply with Chapter 56-A, Title 24 M.R.S. §2302-A, Title 24-A M.R.S.A. §§2749(A), §2846-A and Bureau of Insurance Rule Chapter 850 and Bulletin 265.
