Single Assessment Launch on Feb. 2 - Referral Link for Non-BH Providers

Effective today, February 2, 2026, the Office of MaineCare Services (OMS) and the Office of Behavioral Health (OBH) launched the new Single Assessment process for determining eligibility for medium- to high-intensity behavioral health services. 

External Single Assessment Referral Link 

If a medical provider identifies that a child may need a medium- or high-intensity behavioral health service, they should use the Single Assessment Referral Link to initiate the referral. The Single Assessment is the tool used to determine eligibility, and referrals submitted through the external link go to Acentra. Acentra’s licensed clinicians complete the assessment and determine eligibility on behalf of OMS. 

Intended Use 

The form is not password-protected. The Single Assessment Referral Link is designed for medical providers—not families or the general public. It should be used when you believe a youth may require a medium- or high-intensity behavioral health service and eligibility needs to be determined through the Single Assessment. 

This external link should be used only for children who do not have a BH Care Coordinator or Case Manager, and only by providers who do not use Acentra’s Atrezzo portal. 

BH Case Managers, Care Coordinators, and any provider who uses the Acentra Provider Portal (Atrezzo) should continue submitting Single Assessment requests through Atrezzo.
