Attention Care Coordinators: Atrezzo Coordinated Service Plan (CSP) Replaced by Emailed Supplemental Individual Support Plan (S-ISP) Questionnaire
Effective November 18, 2026, the CSP Questionnaire in Atrezzo will be eliminated and replaced with a new process. Unlike the CSP, which is required for all children receiving Care Coordination, the new S-ISP Questionnaire will only be required for children covered by the Children's Behavioral Health Services (CBHS) DOJ Settlement Agreement.
The new S-ISP Questionnaire takes approximately 10–15 minutes to complete. Care Coordinators can review a preview of the questionnaire to prepare for the change.
How the New Process Will Work
- Questionnaire notification: When a Single Assessment is completed for a covered child, the Care Coordinator identified in the referral will receive an email with a link to the S-ISP Questionnaire.
- Initial questionnaire: The Care Coordinator must complete the questionnaire within 30 days of the first meeting with the child and family to discuss the Single Assessment results.
- Six-month follow-up: Six months after completing the initial questionnaire, the Care Coordinator must complete a follow-up S-ISP Questionnaire. CBHS will send an email to Care Coordinators with a link to the follow-up questionnaire approximately two weeks before it is due. Care Coordinators do not need to track this deadline.
Transition to the New Process
Care Coordinators should continue using the existing CSP process in Atrezzo through November 17, 2026.
Beginning November 18, 2026, the CSP Questionnaire will be removed from Atrezzo. No action is required for existing CSPs.
CBHS will send Care Coordinators an email with a link to complete the new S-ISP Questionnaire for covered children who have completed a Single Assessment before November 18, 2026, and do not have a CSP Questionnaire in progress.
Training
CBHS will hold an introductory training on the new S-ISP process and questionnaire on Friday, November 13, 2026, from 12:00–1:00 PM.
