Cerebral Palsy, Seizure Disorder, and Other Conditions

Overview

If you are 21 years or older, have cerebral palsy, epilepsy, or another condition that is not mental illness and that is closely related to intellectual disability, and are not currently receiving other waiver services, you may be eligible for Section 20 services.

Note: Each service has an assessment to determine an individual’s functional and financial eligibility.

How to Apply

Step 1: Application – Apply by completing the following documents:

  1. 2017-2018 ORC Application (Word)
  2. Authorization-Release (PDF)
  3. ORC Letter of Medical Necessity (Word)
  4. ORC Community Choice Letter (Word)

Step 2: An Office of Aging and Disability Services professional will set up a visit with you to gather information and complete the Health and Safety Assessment. This visit will be face to face. You may request that a family member or other person be present at the assessment.

Step 3: Functional Assessment – If you have not already had a functional assessment by Maine's Assessing Services Agency call 1-833-525-5784.

You will be asked several questions about kind of help a person is looking for and what the financial circumstances are for this person.

Face-to-Face Assessment – Maine's Assessing Services Agency then decides whether a face-to-face assessment is needed. There is no charge, and the assessment is private. If Maine's Assessing Services Agency determines that a face-to-face assessment is needed, you will be contacted by an assessor to schedule the assessment.
